Turn HSE findings into safer work practices

IMPROVE

What will we
do differently
next time?

RISK PREVENTION SYSTEM

Every finding can lead to a change in how work is performed

Improvement becomes tangible when experience changes a control, a work instruction, a pre-job discussion or a training topic. Locstatt connects the tools your teams use to take that next step.

Strengthen the response

Use findings and root causes to guide corrective actions and review task controls.

Make the lesson useful

Bring relevant experience into Lessons Learned, safety meetings and training.

Risk Prevention System

Check the result

Return to inspections, observations and field feedback to see how the change performs.

CONNECTED MODULES. PRACTICAL CHANGE.

One reported hazard. From finding to follow-up.

Follow an illustrative workflow for a recurring pedestrian and vehicle interaction. Each step shows how teams can use Locstatt’s modules together to turn a concern into learning and follow-up.

1. REPORT AND REVIEW

A worker reports a concern

A Hazard Hunt identifies pedestrians crossing a vehicle route. BBS observations and inspection findings help the team understand the conditions around the work.

2. INVESTIGATE & ACT

Address the conditions creating the risk

Supervisors review the findings and assign corrective actions. Depending on the circumstances, the response could include separating routes, improving barriers or changing how work is organized. Incident records can add context from previous events.

3. PREPARE THE NEXT TASK

Bring the change into the work plan.

Review the relevant JSA so the task hazards and revised controls are addressed before similar work begins. Make expectations clear to the people who will carry out the job.

Job Safety Analysis

4. SHARE & APPLY

Share the lesson with the next team

Capture a Lesson Learned, discuss the revised arrangements in an HSE meeting and provide relevant instruction or refresher training. Use the Facility Logbook to review training compliance against configured site-entry requirements.

5. CHECK & REFINE

Return to the field and review the result.

Use follow-up inspections, observations and employee feedback to assess whether the revised arrangements are used, remain practical and address the original concern. Refine the response where further work is needed.

CHECK THE EFFECTIVENESS

Was the action implemented as intended?

Do the people doing the work understand the change?

Does the control work under actual site conditions?

Has the original issue returned—or created a different problem?

HELP EXPERIENCE TRAVEL

One team’s lesson.
Another team’s safer start.

Capture the practical takeaway and bring it to the people who need it. Help other shifts, facilities and teams apply useful experience before they encounter the same issue.

Preserve the context in Lessons Learned.

Discuss the change through HSE Meetings.

Use Facility Logbook to check site training requirements.

Show employees that their reports lead to action

Communicate what changed, why it changed and what the team learned. When employees can see what changed because of their reports, they can understand how their input is used.

SIX STAGES. AN ONGOING PRACTICE.

What you learn today
shapes how you prevent tomorrow.

Continuous improvement brings the cycle back to prevention. Carry forward the controls, knowledge and experience your teams develop, and keep checking how they work in practice.
